You may enter any number of search criteria to narrow down the list.  When you click the Search button, the number of matching providers will be displayed.  If the size of the result set is acceptable, you may click one of the list buttons to produce a printable list of providers.

• Specialty, Practice lists all the providers within each practice.  It is a more compact display.
• Specialty, Provider lists each provider individually, generating a larger list but may be easier to use if searching for a particular individual.

Every effort is made to maintain current and accurate provider data; however, periodic changes to a provider's status can occur.
